BENEFITS
Consumer-Driven Health Care PlanÂ
After six months of employment you are eligible for medical and pharmacy benefits under our new Consumer-Driven Health Care Plan (CDHC). A Health Reimbursement Arrangement (HRA) is established in the name of each associate enrolled in the plan. Medical and pharmacy costs are paid out of this account up to a specified limit, without copayments or out-of-pocket charges. Money in the HRA that is not spent is rolled over to the following year.  If the limit in the HRA is reached then you pay your Share, which is like a deductible amount in most medical plans. After that there is a 70/30 plan, which means the company pays 70% of allowable charges and the associate pays 30%. There are no copayments with this 70/30 portion of the plan either.

Preventive Health Care Services are paid at 100% with no deductions from the HRA. The plan includes help with life-style changes such as smoking cessation and weight management. Other details of the plan include on-line tools and telephone support services. There is also a Personal Health Advisor, one-on-one assistance from a registered nurse to help our associates manage certain health conditions.

Vision Plan
Enrollment in the CDHC entitles you to participate in our vision plan at no additional cost to you. Any family member enrolled in the health plan also has the vision coverage.

Dental Insurance
After you have been employed for 6 months you are eligible for Dental Insurance. You may elect to enroll in the dental plan but not the health plan. You may mix and match your plans (medical only, dental only, or both).

You may receive two cleanings at no charge per year.
Â
Short-Term Disability
Provides 60% of your average weekly wage in the event of an accident or illness that is not work related. Your cost is based on your rate of pay. You may draw benefits for a maximum of 26 weeks after a short elimination period. You will automatically be enrolled after 90 days unless you choose to waive coverage.

Long-Term Disability
Provides 50% of your base salary for a maximum of five (5) years. Benefits are designed to extend beyond your short-term disability coverage, beginning after a 26 week period. Benefits are slightly different for salaried associates. Please contact your local administrative assistant for details.

Cancer Coverage
The plan pays stated benefits directly to you to offset medical expenses and loss of income. Single and family coverage is at the expense of the associate. You are eligible if you are a full time associate and have been employed for six months and meet medical eligibility requirements.

Life Insurance
All associates that are hired and meet their eligibility period will automatically receive Basic Life Insurance. You may also elect additional Supplemental Life and AD&D coverage based upon your age. Coverage is available for spouses and children also.
RETIREMENT PLANS
401(k) Retirement Plan
You may save for your retirement and lower your current income taxes with tax-deferred contributions. You can contribute up to 15% of your gross pay with convenient payroll deductions. Bonitz will contribute an amount equal to 20% of the first 5% of your contributions. Our 401(k) plan is managed by one of the leading providers who offers a wide range of funds for you to choose from. You are eligible to participate once you have worked for Bonitz for one year with at least 1000 hours worked during that year. You may access your account by phone or Internet 24 hours a day.Â

ESOP Employee Stock Ownership
Bonitz is an employee-owned company. You will automatically receive Bonitz Stock, at NO cost to you, after completing one year of service with at least 1,000 hours worked during that period. To receive stock thereafter, you must work at least 1,000 hours and be employed by Bonitz on December 31st.
Special Programs
Safety Incentive
If your profit center has no lost-time accidents during a six-month period (January 1 through June 30 and July 1 through December 31), and if you have worked at least 960 hours during this six month period and have not had any work-related accident requiring medical attention for these six month periods, you will receive a cash award.

Belt Buckle Program
Associates who work for Bonitz one year without a lost-time work related accident, receive a belt buckle. For succeeding years with no lost-time accidents, they receive a corresponding number to insert into the belt buckle. For those associates who go five years without a lost-time accident, they receive a Bonitz windbreaker. For 10 years, a Bonitz jacket and for 15 years, a Bonitz heavy coat.

Bonitz Bucks
20% of the profit on savings over the original cost of each project is put into the Bonitz Bucks program. Each associate's share is based upon the number of hours he or she worked on that project.

Holidays
You are eligible for the following six paid holidays after the completion of one year of employment: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.

Payroll Direct Deposit
You may elect to have your paycheck directly deposited into your bank account. A form is available during your orientation.

Quality Awards
You have a variety of opportunities to earn Quality Awards. Generally, these awards may be recommended to teams when your performance exceeds company and/or customer expectations.

Vacation
After you have worked for Bonitz for one year, you are eligible for vacation.

Employee of the Month, Employee of the Year
The Selection Team at each Bonitz location considers nominations for employee of the month and determines monthly winners. Each monthly winner receives an award. Employees of the Month are eligible for the Employee of the Year award.

Bill Rogers Bonitz Scholarship Fund
The Scholarship Fund will award one scholarship each year to a child of a full-time associate of Bonitz. The recipient must meet Scholarship guidelines established by the Fund. The program is administered by an independent nonprofit organization.

Annual Awards Banquet
For every five years of service, you will receive an award, which you may choose from a catalog of gifts. Your years of service will be recognized at the Annual Awards Banquet.
